The Post UTME is the school’s own screening, sat after your UTME result is out. JAMB neither sets it nor marks it — Kwara State University decides the subjects, the number of questions, the clock, and how heavily the score counts.
Most schools publish the same arithmetic: (UTME score ÷ 8) + (screening score ÷ 2), which gives a figure out of 100, half of it your UTME and half of it the screening. Not all of them, though.
